Registration
All the patients taking treatment in HCG Medisurge
Hospitals for the first time have to get registered,
giving all their demographic details. Upon registration,
patient is given a Unique Identification Number (UIN)
and an Identity card of the hospital. The patient would
also be given an ID band which is to be worn at all
times while in the hospital. The identification number
is important as it would be required for any services or
assistance from hospital during the stay and beyond.
Admission Planning
For all planned admission, we recommend you to book and
confirm your room in advance.
Admission for surgery - we advise you to get
admitted a day prior to surgery to facilitate all
necessary investigations and medical evaluation.
Things to bring at the time of
admission
• All your medical documents related to admission
• List of all current medications
• Required documents for Third Party Administrator (TPA)/
Corporate authorization
We request you not to bring any valuables like jewellary.
Hospital management is not responsible for loss of any
valuables. You would be provided with toiletries like
soap, tooth brush and tooth paste.
Admission Procedure
At the time of admission please report to admission
department (ADT) at ground floor. You need to fill up
your details to get registered as an in-patient. You
will also be required to sign consent for treatment and
financial obligation.
Your billing class would depend upon the kind of room
category you select. The admission staff will explain
you about the room category, the type of room, services
and the respective charges.
You would be required to pay admission deposit as per
the room category selected.
However, a deposit of Rs. 5000/- only is taken at the
time of admission if you have cashless facility. This is
for non-medical expenses like registration and service
charges that are not paid by the insurance companies.
Medical Insurance & Third Party
Administrator
Our Helpdesk executive would assist you if you wish to
avail cashless facility. You have to submit a copy of
current year’s Health Insurance policy, TPA card, copy
of photo identity proof of the patient (Passport,
Voter’s card, Driving license, PAN card, etc.) and any
other documents required by TPA. You are also required
to get pathology/ radiology or any other investigation
reports which point out positive report for the present
ailment.
The current year’s policy is mandatory, but you are
requested to get copies of last few years’ policy
documents; these will help in faster processing of the
insurance.
Your cashless facility would depend on the authorization
received from the TPA. |
